The Lead Camp Nurse, is responsible for the overall health and safety of all campers and staff. Residents are encouraged to actively engage in their communities through volunteering, education, part-time work, and maintaining connections with family and friends.</p><p data-start="1301" data-end="1527">The home provides a safe and secure environment with 24/7 staffing, including a dedicated medical team. Residents’ conditions vary and currently include Cerebral Palsy, Muscular Dystrophy, Multiple Sclerosis, and Spina Bifida.</p><p data-start="1529" data-end="1553"><strong data-start="1529" Service Navigation</strong></p><p>• Provide clear, accurate, and accessible information on disability supports and services, including Persons with Developmental Disabilities (PDD), Family Managed Services (FMS), and Family Supports for Children with Disabilities (FSCD) agreements.</p><p>• Support individuals, families, and professionals in navigating systems, accessing resources, and coordinating services to meet their unique needs.</p><p>• Guide clients through needs-based assessments and co-create personalized service plans that promote independence, inclusion, and wellbeing.</p><p>• Connect clients to community and provincial resources, including financial supports, adaptive equipment, mental health services, and other disability-related programs.</p><p>• Assist clients in accessing and using digital tools (e.g., OCR system, Alberta.ca accounts, digital signatures) to support efficient service access.</p><p><strong>Capacity Building &amp; Education</strong></p><p>• Empower clients and families to confidently manage their supports by providing coaching on employer responsibilities, including hiring, training, and managing staff.</p><p>• Offer guidance on financial reporting and compliance requirements for FMS and FSCD agreements.</p><p>• Deliver engaging learning opportunities for clients, families, and professionals on topics such as FSCD and PDD programs, employer responsibilities, Financial Literacy, and best practices.</p><p><strong>Program Delivery &amp; Continuous Improvement</strong></p><p>• Support the development and implementation of key planning tools such as Individual Support Plans (ISPs), Transition to Adulthood Plans (TAP), Behavioural Support Plans (BSPs), and related processes.</p><p>• Monitor program performance, track outcomes, and gather client feedback to identify trends, challenges, and opportunities for improvement.</p><p>• Translate insights and data into actionable recommendations that enhance service quality and client outcomes.</p><p>• Develop and implement processes that improve workflow efficiency and strengthen service delivery.</p><p>• Ensure programs align with Easter Seals Alberta’s strategic plan and commitment to continuous improvement.</p><p><strong>Partnerships &amp; Advocacy</strong></p><p>• Integrate inclusive practices into all aspects of service delivery.</p><p>• Actively address barriers to ensure equitable access for all individuals and families.</p><p>• Support clients in building natural supports and meaningful connections within their communities.</p><p><strong>Required Qualifications:</strong></p><p>• Post-secondary education in Social Work (or an equivalent combination of education and relevant experience).</p><p>• Minimum of 2–3 years’ experience in case management, service navigation, or supporting individuals and families in a human services environment.</p><p>• Demonstrated knowledge of Alberta disability support systems, including PDD, FSCD, and FMS, or the ability to quickly build this expertise, is an asset.</p><p>• Experience working with individuals with disabilities and/or chronic medical conditions, with a strong understanding of person- and family-centered approaches.</p><p>• Strong communication skills (written and verbal), with the ability to explain complex information clearly, compassionately, and accessibly. Individuals with skills in more than one language would be an asset.</p><p>• Proven ability to build relationships and work effectively with diverse individuals, families, professionals, and community partners.</p><p>• Experience facilitating training, workshops, or educational sessions for clients, families, or staff.</p><p>• Strong organizational and time management skills, with the ability to manage multiple priorities in a fast-paced environment.</p><p>• Demonstrated problem-solving skills, sound judgment, and the ability to take initiative in addressing client and program needs.</p><p>• Ability to analyze information, identify trends, and contribute to continuous improvement of programs and services.</p><p>• Proficiency with Microsoft Office and digital tools (experience with SharePoint or similar platforms) is an asset.</p><p>• Experience developing and implementing Individual Support Plans (ISPs), Transition to Adulthood Plans (TAP), and Behavioural Support Plans (BSP’s) is an asset.</p><p>• Commitment to diversity, equity, inclusion, and belonging, with the ability to apply these principles in practice.</p><p>• Ability to work both independently and collaboratively within a team environment.</p><p><strong>Position details:</strong></p><p>• This position is 40 hours/week fully remote with mandatory team meetings in-person within the Calgary office occasionally.
